Forms for Schools (insured date)
Below are the forms required for schools planting sites on an insured date.

For sites on insured dates it's important to have this form on-hand in case you need to report any accident or incident to Planet Ark. - Photo Release Form (87kb pdf file)
If you plan to upload photos of your site, or for some competition entries, you will need to have anyone that can be identified in the photos sign this form. - Schools - Additional Volunteer Registration Form (193kb pdf file)
Use this form for signing up any EXTRA volunteers (eg parent helpers) that are not already registered with the school to ensure everyone is covered by insurance. We DO NOT need to have students fill out these forms since you will already have a class roll and have permission for students to take part from parents (this is the school's responsibility). - Site Results Registration Form (150kb pdf file)
