For sites on insured dates it's important to have this form on-hand in case you need to report any accident or incident to Planet Ark. (300kb pdf file) - Photo Release Form

Use this form for signing up any EXTRA volunteers (eg parent helpers) to ensure everyone is covered by insurance. We DO NOT need to have students fill out these forms since you ewill already have a class roll and have permission for students to take part from parents (this is the school's responsibility).
